LeadingAge Massachusetts member organizations may submit a proxy vote electronically through DocuSign.
One proxy vote per member organization. The proxy form should be completed by the person submitting the vote on behalf of the member organization.

Members are encouraged to review the merger materials before submitting the electronic proxy form.
The materials include information about the proposed merger, the Merger Agreement and Plan of Merger, proposed amendments to the Articles of Organization, and proposed bylaws for the combined organization.
